Jennifer Miles, PharmD, BCACPProgram Director
Dr. Jennifer Miles is the Clinical Coordinator for the Baptist Health South Florida Clinical Pharmacy Enterprise Ambulatory Pharmacy team and also serves as the Clinical Pharmacist supporting the Value-Based and Population Health management team in the Baptist Health Quality Network (BHQN). Since joining the health-system in 2015, Dr. Miles has worked with physicians and staff members in ambulatory practice on clinical program development and quality outcomes for high-priority patient populations.
Dr. Miles is a graduate of Nova Southeastern University College of Pharmacy, where she received her PharmD. She completed a PGY-1 Pharmacy Practice Residency at the Miami VA Healthcare System. Dr. Miles holds the distinction of being a Board Certified Ambulatory Care Pharmacist. She has certifications in Immunization Administration as well as being a Trainer, Medication Therapy Management (MTM) Specialist and Pharmacogenomics Specialist. Prior to joining BHSF, Dr. Miles served as Manager of HIV/AIDS and Hepatitis Centers of Excellence in the North Miami area and oversaw a hospital discharge team. Her areas of focus now include Primary Care, Diabetes patient management and Cardiology Pharmacy—with specific concentration on lipid management, refractory hypertension and Cardiometabolic patient management.
In addition to her professional pursuits, Dr. Miles serves as a Clinical Affiliate Assistant Professor for Nova Southeastern College of Pharmacy. She is a preceptor for: APPE students from Nova Southeastern University (NSU), Florida Agricultural and Mechanical University (FAMU) and Palm Beach Atlantic University (PBA); the BHSF PGY-1 Residency Program Ambulatory Pharmacy rotation; and Nova Southeastern PGY-2 Behavioral Health Pharmacy Residency program for an elective course. Dr. Miles is a member of the South Florida Society of Health-System Pharmacists (SFSHP), The American Society of Health-System Pharmacists (ASHP), Dade County Pharmacy Association (DCPA) and the National Lipid Association (NLA).
Michael A. DeCoske, PharmD, BCPSAssistant Vice President of Ambulatory Pharmacy Service
Dr. Michael DeCoske is the Assistant Vice President of Ambulatory Pharmacy Service for The Baptist Health South Florida Clinical Pharmacy Enterprise. Since joining the health-system in 2018, Dr. DeCoske has overseen the expansion of Baptist Health South Florida’s Pharmacy footprint into the ambulatory segment of clinical services.
Dr. DeCoske is a graduate of Duquesne University, where he obtained his PharmD. Dr. DeCoske completed a PGY1 and Health System Pharmacy Administration (PGY2) Residency at the Medical University of South Carolina in Charleston, South Carolina. Prior to this role, he served as Associate Chief Pharmacy Officer for Ambulatory Services at Duke University Hospital in Durham, North Carolina. He also holds distinction as a Board Certified Pharmacotherapy Specialist.
Dr. DeCoske is committed to professional pharmacy involvement. He currently serves as Chair of the Section of Specialty Pharmacy Practitioners Business Development Section Advisory Group. He is a co-author of the ASHP Pharmacy Leadership Field Guide.
Fernando J. Zaldivar, BPharm, RPh, CRPhCorporate Director of Business Development and Trade Relations
Fernando Zaldivar is the Corporate Director of Business Development and Trade Relations for The Baptist Health South Florida Clinical Pharmacy Enterprise. Zaldivar joined the health-system in 2018, and through his vision and leadership, is at the helm of expansion of pharmacy services by guiding operational and Managed Care collaborations between the Clinical Pharmacy team with members of the health-system as well as community partners.
Zaldivar is a graduate of University of Florida where he received his Bachelor’s of Science in Pharmacy with Honors, and of Boston University where he completed a Leadership Program. He holds numerous certifications—including Management of Quality Improvement in Health Care and Administration Hospital Process Management and Improvement. Zaldivar has served as a healthcare leader with several well-known, large-scale organizations—including having served as Director of Pharmacy for Care Plus, Cardinal Health, HCA East Florida and Memorial Healthcare System and has been a Vice President of Pharmacy Services for Cano Health. He also brings Managed Care Pharmacy experience, previously serving as Corporate/Market Director of Pharmacy Operations for Humana. His focus areas for BHSF include Managed Care, Population Health and Contracting.
Through the course of his career, Zaldivar has been an author on research papers examining a variety of focuses—from pricing programs for Ryan White Title I Drugs, the role of pharmacy in the continuum of care as well as pharmacist integration as part of multidisciplinary healthcare teams. He has been recognized with distinctions for his service to our profession by ASHP, SFSHP and Florida Pharmacist Association (FPA).
Angelica A. Berni, PharmD, MS, BCPS, CPhDirector, Specialty Pharmacy Services
Dr. Angelica Berni is the Director of Specialty Pharmacy Services for Baptist Health South Florida. Dr. Berni joined the BHSF team in 2018 and has overseen the expansion of specialty medication supply chain across the outpatient segment of the health-system during her tenure.
Dr. Berni is a graduate of Palm Beach Atlantic University College of Pharmacy, where she received her PharmD. She also holds a Master’s of Science in Pharmacy Practice/Administration. Dr. Berni completed a dual PGY-1/PGY-2 Residency program at The University of Kansas Health system in Health System Pharmacy Administration. She is a Board Certified Pharmacotherapy Specialist, has a Consultant Pharmacist license in the State of Florida and a Preceptor Training Certification.
In addition to her professional accomplishments, Dr. Berni participates as a member of a number of pharmacy-facing organizations—including The National Community Oncology Dispensing Association (NCODA), The Florida Society of Health-System Pharmacists (FSHP) and The American Association of Health-System Pharmacists (ASHP).
Faaria Quadri, Pharm.D., BCPSClinical Pharmacy Specialist, Ambulatory Care Pharmacy
Faaria Quadri, Pharm.D., BCPS, received her Bachelor of Science degree in Biology from Nova Southeastern University (NSU) in 2007 and continued on at the NSU College of Pharmacy where she obtained her Doctor of Pharmacy degree in 2011. She completed a PGY-1 Pharmacy Practice Residency at the University of Chicago Medical Center and subsequently started her career as an Anticoagulation/Transitions-of-Care Pharmacy Specialist in Brooklyn, New York. Faaria joined Baptist Health in 2018 as a Cardiology Clinical Pharmacy Specialist at BHM and participated as a pharmacist member in several Baptist Hospital, Baptist Health and Miami Cardiovascular Institute (MCVI) Committees. She also served as a preceptor for both pharmacy residents and students from NSU and University of Florida for Cardiology and Internal Medicine rotations and holds certifications in anticoagulation management and medication therapy management.
